Why patients don’t turn up

A no-show is rarely a careless patient. In most Indian OPDs it is the predictable result of how the visit was arranged. The patient was told “come in the morning”, not given a sitting. They were not sure they would actually be seen. Nothing was paid, so nothing was lost by not coming. The journey was long and the waiting room was longer. Somewhere between the phone call and the clinic door, the visit quietly stopped being worth it.

That is good news, because every one of those causes is something the clinic controls. The seven changes below are the ones that move the number in practice. None of them needs a bigger team.

Seven ways to cut no-shows

1. Let patients pay when they book

A paid seat is a kept seat. When the consultation fee is paid by UPI at the moment of booking, the patient has already made the decision to come, and the clinic has already been paid for the slot either way. In the clinics running on Treatlly, no-shows on prepaid online bookings are rare. Your refund and cancellation rules stay yours; publish them so the fairness is visible.

2. Give a sitting, not a vague morning

“Any time before 1” invites patients to arrive at 7 and leave at 11. A defined sitting (say 9:00–12:00, 30 seats) with a seat number gives the patient something concrete to plan around. Keep the sittings honest: if the doctor usually starts at 10, do not publish 9.

3. Confirm instantly, in writing

A confirmation SMS with the date, the sitting and the clinic name turns a conversation into a record. The patient can show it at the counter and a family member can read it. Reschedules and cancellations should go the same way, automatically, so nobody has to ring the clinic to find out whether a holiday affected them.

4. Make rescheduling easier than disappearing

Patients who cannot come usually know a day in advance. If moving the visit means a phone call that nobody answers, they simply do not come. A reschedule link that works from any phone turns a no-show into a visit on another day, and frees the seat for someone else.

5. Publish fair cancellation rules

Decide what happens when a patient cancels the night before, the same morning, or not at all, and write it on the booking page. Rules that patients can read are rules they respect. Keep them humane: a patient who cancels a day ahead should not lose the fee.

6. Book the follow-up before the patient leaves

Follow-ups are the visits most often forgotten. Set a follow-up window per doctor (for example, free within 10 days) and let the patient pick the day while they are still at the counter, or from home when the window opens. A follow-up with a date is a follow-up that happens.

7. Run a queue patients trust

Patients stop coming to clinics where arriving on time does not help. A token given at check-in, a list that follows arrival order, and an honest message when the doctor is running late (“pushed by 30 minutes”, sent to everyone waiting) keep the promise the booking made. Trust compounds: the patient who was seen in order books again.

Measure it before you change it

Most clinics guess their no-show rate and guess high. Count it for two weeks: booked visits, visits that happened, visits cancelled in advance, visits that simply did not arrive. Separate new patients from follow-ups and online bookings from counter bookings; the rates are usually very different. Then change one thing at a time. Prepaid booking alone usually moves the online number more than everything else combined.

Treatlly shows each doctor today’s load, the new-versus-follow-up mix and the no-show trend, so the count is there without a spreadsheet. See how an OPD runs on Treatlly and how prepaid online booking works.

Frequently asked questions

Should a clinic charge a no-show fee?+

A prepaid consultation fee does most of the work without a separate penalty: the seat is paid for whether or not the patient comes, and your refund rules decide what happens next. If you do charge a fee to re-book after a no-show, keep it small and publish it on the booking page.

Do SMS reminders reduce no-shows?+

A confirmation SMS at booking time, with the date and sitting, is the message that matters most, because it is the one patients keep and show. Treatlly sends confirmations, reschedules and cancellations by SMS automatically; a reminder SMS before a video consultation goes out 15 minutes before the call.

What about patients who book online and still walk in early?+

Give them a token at check-in like everyone else. Their seat is already held, so the token simply records the arrival order within the sitting. Over a few weeks, patients learn that arriving at the sitting time works, and the early crowd thins.

Does prepaid booking put patients off?+

Not when the fee is the same one they would pay at the counter and the refund rules are clear. UPI makes the payment a few seconds long. Clinics on Treatlly see the large majority of their visits start with an online, prepaid booking.
